Engaging Patients with the Well-Visit Planner
As a finalist of AcademyHealth's Relevant Evidence to Advance Care and Health (REACH) Challenge, the CAHMI and DRC will be presenting a new and innovative tool designed to customize, tailor and improve the quality of well-child care for young children. This evidence-based tool engages parents as proactive partners in planning and conducting well-child visits using a simple three-step process. The Well-Visit Planner also has the capacity to integrate clinically-relevant information directly into the EHR. Check out this quick video and one-pager and visit www.WellVisitPlanner.org for more details.
Come see the Well-Visit Planner in action at the Innovation Station on the lobby level of the Dolphin Hotel on Sunday and Monday morning. Also join a discussion on "the Role of HSR in Stimulating Innovation", Tuesday
at 11:30am in the Dolphin Hotels's Southern Hemisphere 1 Room.
The Data Resource Center for Child and Adolescent Health will be at Booth 410 for the duration of ARM. Come say hello.
