Announcing the creation of
The Coastal Community Care Team

Dear patients and friends,

Seaport Family Practice has joined with Blue Hill Family Medicine and the Community Health Center in Southwest Harbor to create the Coastal Community Care Team.


What exactly is a Community Care Team (CCT)?
A CCT is a collection of professional healthcare providers including doctors, nurses, health educators, behavioral therapists and social workers who are available to provide additional healthcare services to patients identified by their doctor to be at a higher risk for complicated health problems.

The Coastal CCT will offer individually tailored healthcare services and education designed to improve the health and quality of life for some individuals who may benefit from a higher level of care. These services may be provided in the doctor's office or in the patient's home.

These alternative healthcare services will be in addition to the traditional office based visit and will be offered  at no additional cost to the patient.  CCTs also work to coordinate and connect patients to alternative community healthcare providers in order to support their health improvement goals, achieve better health outcomes and reduce avoidable costs.  


Background of the CCT and its relationship with the Patient Centered Medical Home Pilot program:

In 2010 Seaport Family Practice began participating with the Maine Patient Centered Medical Home (PCMH) Pilot, an innovative program for improving primary care. PCMH gives practices information about organizing care around patients, working in teams and coordinating and tracking care over time.

As a Patient Centered Medical Home, Seaport Family Practice has committed to becoming a health care setting that facilitates partnerships between individual patients, and their personal physicians, and when appropriate, the patient's family. 

The CCT was created by the Maine PCMH Pilot as a key strategy for improving care and reducing avoidable costs for patients in the Pilot, especially those with complex or chronic conditions.

At Seaport Family Practice we are committed to helping our patients access needed healthcare services while reducing avoidable costs. The CCT is one more way that we are able to do that.
