Inspired by the driven women in Dr. Rubino's personal life who have raised families, ran households and thrived professionally, he has created this section of the website to provide the platform for our patients to network their skill set, products, services and talents. | PID - A Personal Perspective |
Stacey is a college junior who recently started a new relationship with Roger,  her boyfriend. Two days ago, she started feeling some lower abdominal cramping, that intensified despite taking a few motrin. She woke up yesterday with a fever and chills. After a visit with her gynecologist she was diagnosed with P elvic Inflammatory Disease (PID), given a course of antibiotics, and started feeling better in a couple of days. Stacey wanted to know more, so she made a follow-up appointment after her google search scared her. After speaking at length with her gynecologist, here is what she found:
The recent onset of pain that worsens during intercourse or with jarring movement may be the only presenting symptom of PID; the onset of pain during or shortly after menses is particularly suggestive. The abdominal pain is usually on both sides and rarely more than two weeks in duration. Abnormal uterine bleeding occurs in one-third or more of patients with PID. New vaginal discharge, fever, and chills can be associated signs. Some women are at increased risk for PID if they have a sexually transmitted infection (STI). Risk factors for STIs include:
- Age less than 25 years
- Young age at first sex
- Non-barrier contraception
- New, multiple, or symptomatic sexual partners
- Oral contraception
Factors that potentially facilitate PID include:
- Previous episode of PID
- Sex during menses
- Vaginal douching
- Bacterial vaginosis
- Intrauterine device
Most cases of PID can be treated as an outpatient, but some cases can require hospital admission and in rare cases, surgery to remove abscess. The effects of the infection can damage future fertility. Preventiative measures that can be taken to reduce the incidence of PID are, using condoms and limiting the number of sexual partners.

Healthy Living: Fiscally Sound Health Excerpt from patient Monika Mahajan's blog | |
My husband and I were recently discussing household expenses and how difficult it is to stay within budget. At one point, he stopped trying to justify the costs but instead remarked how saving money was like losing weight - not easy but possible with the right discipline and strategy. That got me thinking about this analogy and how far it could be extended.
Weight loss and fiscal discipline both depend a lot on your habits. The modern urban lifestyle provides several opportunities to deviate from the norm - whether its managing portion sizes or a budget. Do I spend beyond my means on non-essential shopping? Or how often and how much do we eat out? My choices decide how successful I am at saving money or eating a healthy diet, and the consequences. The more I spend unnecessarily, the less money I would have and more loans I may need. Similarly, the more unhealthy the food I eat, the greater chance I might end up with heart disease, high blood pressure, weight gain or worse.
I can also expand this analogy to portion sizes and mortgages. Just because there is an "all you can eat" buffet does not justify binge eating, even if skipping breakfast. Hunger does not correlate to justifying 5 meals at once. Eating slowly and savoring your food and stopping eating when full, can help avoid over-indulgence. Likewise, with mortgages, there is always temptation to borrow as much money as the lender is willing to lend - especially if it is accompanied with attractive financing options! However, a loan is still a loan and the bigger the loan, the more likely to introduce financial stress on your family, hence jeopardizing financial health. Realizing limits and only accepting a loan that is within a monthly budget allows one to breathe comfortably.
Any change is hard to accomplish and even harder if it involves changing behavior. Baby steps, incorporating one adjustment at a time, can make a big difference. Eating healthier, integrating more greens, less meat and locally grown and organic rather than processed foods is a great start. Exercising regularly - no need for a fancy gym - walking, running or taking the stairs is a step in the right direction.
If watching a budget, slowly eliminate the everyday luxuries and, instead, incorporate rewards once a week with a small splurge. If I challenge myself - eliminating that Starbucks latte every day - I can save $100 a month or $1,200 a year! (not to mention consuming 200 calories less every day or 1,400 calories/wk - nearly half a pound!).
Ultimately, I realized, in both finances and weight reduction, it is a numbers game, consuming more calories than I expend (even healthy food) leads to weight gain and spending more than I take home in cash, will grow my debt not my bank account. |

This month we highlight Nancy Francischetti, practice advisor, receptionist and assistant biller for The Rubino OB/GYN Group. 
For Nancy, working for Dr. Rubino is truly a family affair. She is Dr. Rubino's oldest sibling and is married to Paul, the director of operations for the Rubino Group. She can also be seen greeting patients at Primary Care Physicians Group where she works for her sister-in-law, Dr. Susan Rubino.
Nancy's started working with the Rubino Group after moving back to the NJ area from North Potomac , MD. After moving back "home", Nancy entertained the thought of returning to the classroom. An elementary teacher with a masters degree, she taught at Washington School in Nutley for six years before staying home to raise her family. When Dr. Rubino suggested working for him at the front desk she accepted his offer. The "career change" was a smooth transition for her as she so enjoys the interaction with the patients. She is particularly fond of seeing the newborns come into the office and is thrilled to "babysit" on occasion while the new mom has her appointment . It is a privilege for Nancy to work in a caring environment that exemplifies the Rubino Ob/Gyn Group atmosphere.
Nancy and her husband, Paul, have been married for 35 years and have two daughters. Carla and her husband, Matt live abroad in the Netherlands . Renee, a pharmaceutical rep, has also worked for Paul in the billing department. Surrounded by family and the wonderful people of the Rubino Ob/Gyn Group , going to work every day is an extension of home for Nancy.
Dr. Rubino credits Nancy with coming up with many quality of life improvements for how to run the day to day. Not to mention that she fed him and his roommates regularly during medical school when they had no time or money. Nancy is an integral part of the practice. |
