Medical Fact

 "I didn't know that!"
 In 1920, Dr. Papanicolaou began his study of the vaginal cytology of the human. After becoming familiar with normal cytology changes, he found some cases of malignancy in a smear of the uterine cervix. In 1928 he published a paper entitled, "New Cancer Diagnosis."  In 1943, his findings were published in the famous monograph, "Diagnosis of Uterine Cancer by the Vaginal Smear." The diagnostic procedure was named the "Pap test" and has since been administered to millions of women, greatly reducing and preventing deaths from cancer of the uterus.

What is a Pap Smear?
The pap smear (named after inventor, George Papanicolaou) has become the most successful cancer screening test in the history of medicine. 

Cervical cancer was the leading female cancer in the 19th century, leading to further research in gynecology to detect premalignant and malignant (cancerous) processes in the cervix. It was realized significant cell changes, visible with a pap smear, can be treated, thus preventing cervical cancer.  In 2000, the "Thin Prep Pap Smear", a liquid based technology, was approved by the FDA as more accurate than the traditional or conventional Pap smear.

The "pap" is generally performed once per year for women, at reproductive age and older, at their annual check-ups. It is designed to pick up indications of abnormal cell growth very early, well before they become a problem.

The majority of dysplasia goes away on its own, but in a small portion of patients with dysplasia, it can progress.  Patient's with an abnormal pap smear may be brought back to the office to have a visual test, called a colposcopy, to look more closely at the cervix. A small tissue sample may be taken from the cervix during a colposcopy. In some cases, patients may be advised to get a leep procedure, which skims off a small portion of the problem area on the cervix, in hopes of removing the problem. After an abnormal pap or a leep procedure, patients often are put on a more frequent surveillance program than the typical once per year recommendation.

Importance of the Flu Shot and Tdap Vaccines
Flu Vaccine
The single best way to protect yourself against the seasonal flu each year is to get a flu shot.

The "flu shot" is an inactivated vaccine (containing killed virus), typically administered in the arm for people older than 6 months, including healthy people and people with chronic medical conditions.  

While everyone should get a flu vaccine each flu season, it's especially important that the following groups get vaccinated: 

1. Pregnant women
2. Children younger than 5, but especially children younger than 2 years old
3. People 50 years of age and older
4. People of any age with certain chronic medical conditions
5. People who live in nursing homes and other long-term care facilities
6. People who live with or care for those at high risk for complications from flu, including: health care workers, household contacts of persons at high risk for complications from the flu and out of home caregivers of children less than 6 months of age (these children are too young to be vaccinated)

Pregnant women should not receive the nasal spray flu vaccine. 

Tdap Vaccine

According to ACOG (The American College of Obstetricians and Gynecologists) the Tdap vaccine is recommended for protection against pertussis, tetanus or diphtheria. Tdap is a combination vaccine for all 3 of these diseases.

ACIP (Advisory Committee on Immunization Practices) guidelines on administration of these vaccines:

 
 

ˇ     Pertussis is still endemic in the U.S. and persons between the ages of

     11-64 should be administered the vaccine, once during a lifetime

      ˇ   Th
e Tdap is recommended to women before conception if the woman  
                     has not received Tdap, and if at least 2 years have passed since her last
                     Td booster

ˇ     Pregnant women who have not received a dose of Tdap previously

      should receive Tdap after delivery and before discharge from the

      hospital if 2 years or more have elapsed since the most recent Td 

      administration; however the Td booster should be given during

      pregnancy if 10 years or more have elapsed since a previous Td

      booster

ˇ       For added protection against pretussis or for pregnant women who

       need tetanus or diphtheria protection during pregnancy, vaccination

      with the Tdap instead of Td can be administered in the second or

      third trimester

 

Benefits of vaccinating pregnant women usually outweigh potential risks when the likelihood of disease exposure is high, when infection would pose a risk to the mother or fetus, and when the vaccine is unlikely to cause harm. No evidence exists of risk from vaccinating pregnant women with inactivated virus or bacterial vaccines or toxoids.

Healthy Living: Eat Your Vitamins
Two newsletters ago, we high-lighted six essential vitamins and minerals (Vit A, B, C, D, Calcium and Iron) that promote healthy living, from the inside out. We are often asked during an appointment what is the best vitamin or supplement to take to ensure the most important vitamins and minerals are part of a daily diet.

The best vitamins and minerals available are in the natural foods that you eat. Natural foods provide an abundance of nutrients and fiber that are not available in pill form. Non-processed, naturally derived and naturally grown foods provide the most beneficial vitamins and minerals available.

The nutrients found in these foods are what keep your body functioning at its best - building strong bones; improving brain power, mood, and memory; and possibly helping the immune system ward off ailments both small (a cold) and large (cancer).

The best guide when trying to eat your vitamins is to shop the outer aisles of the supermarket, avoid the middle aisles which are often filled with manufactured and processed foods. Fruits and vegetables offer some of the best defenses for healthy living. To get those essential vitamins and minerals, it is recommended that you incorporate the following foods into your daily diet:

Whole grains (breads, cereal, brown rice), legumes/nuts, lowfat dairy, lean meats, fish high in omega 3's (tuna, salmon), colorful fruits and vegetables, greens, wheat germ/flaxseed and healthy oils (olive).

Ingesting a diet full of these foods will help you eat your vitamins naturally, giving you the best fuel for living a healthy life.

This month we highlight Chris Fordyce, receptionist for The Rubino OB/GYN Group.
Chris has been with The Rubino OB/GYN Group for 5 years. She keeps busy working on a variety of different tasks and projects for Dr. Rubino while working as a receptionist in the Summit and West Orange offices. Chris also keeps Dr. Rubino running on time and updates him on Yankee gossip.
Chris enjoys spending time with her family, going out with friends, listening to music and going to Yankee games. Her greatest joy is being the proud mom to her son Stephen.
