Health Care Transition Workshops |
|
It is important for families to begin thinking about their child's transition from pediatric health care to adult health care services. This workshop will provide parents, families and youth with special health care needs information and ideas on how to make the health care transition as smooth and worry free as possible. There will be two workshops available one for the youth and one for parents. There will be eight workshops held across the state over the next several weeks, with the first two beginning this week. Please note the dates and times below.
