Capital BlueCross: Provider Directory Changes
In an effort to move toward a "greener environment" and to
respond to frequent customer requests for an electronic version of our provider
directory, we are creating a CD version of the commercial provider directory.
This initiative will take effect beginning with the next directory release in
April 2009. The "Go Green" initiative was first introduced in our Medicare
marketing materials in late 2008 when we gave potential members a CD rather
than a paper directory, with an option to receive a paper version if necessary.

colorectal cancer screening. It affects insured employers with 51 or more
employees.

United Healthcare: Pharmacy Drug List Changes
Effective May 1, 2009, several medications will move tiers on our Advantage and Traditional Prescription Drug Lists (PDLs). Up to six times a year, up-tiering changes are made to our Advantage PDL. On January 1 each year, we synchronize the Traditional PDL with the Advantage PDL up-tiering changes that were made throughout the prior year. Down-tiering and benefit plan changes occur throughout the year on both PDLs.
