New 2009 Signature Form Now Available
In response to Medicare's new Signature Rule that goes into effect January 1, 2009, PWW has made available - at no charge - a new Signature Form for all ambulance service providers and suppliers.
This new form is designed to assist ambulance services in complying with applicable Medicare signature requirements. You may download the form and customize it for your agency's use. Instructions for the new form are also provided.
To view the sample signature form, please CLICK HERE. In light of recent commentary from CMS, as part of the November 19, 2008 "Final Rule," PWW feels it is no longer necessary to utilize two different forms for suppliers and providers, as the distinctions on the forms are minor.
