'Tis the Season for Giving

At this time of year many of us think of giving to others. And no doubt one of those recipients will be your baby. The greatest gifts to your little person are the gifts of yourself - nurturing, communication and play.

If your baby is soon to be born, now is the time to think about what your baby wants at birth. Here's what Teresa Pitman, author and former executive director of La Leche League Canada had to say at a recent conference.

She suggests sitting up and holding your baby skin-to-skin immediately after birth. Enjoy your baby with all your senses as your baby comes to know you on the outside. Gaze into your baby's eyes and speak words of welcome and love to your little one. Your partner can also stroke and welcome the baby.

This immediate skin-to-skin contact on the outside maintains the baby's body temperature, stabilizes the baby's heart and breathing rates, prepares the baby to find the breast, stimulates the mother's milk production, and prevents the baby from becoming stressed.

Your baby wants to breastfeed and knows how.  Often in the first hour, sometimes in the first 30 minutes and sometimes later, your baby will begin to show interest in suckling. She may stick out her tongue, open her mouth as though she is looking for something, move her head from side to side, perhaps suck on her hands and then begin to bob her little head toward the breast. Then all you need to do is shift the way you are holding her so that she can find her way. Most babies will then self-attach (latch on without any effort on the mother's part) and latch remarkably well.

Your baby will likely come to the breast more easily if you have not received medications in labour and if he has not been suctioned after birth.

Make your baby's arrival as stress free as possible. Skin-to-skin contact with you is the baby's "natural habitat." Plan to spend as much time as possible with your baby next to your skin in the early days after birth. You'll be ready to respond to your baby's need for food immediately. Responding to your baby's cues will strengthen your bond of love and trust. Your baby will have no need to cry so will come to the breast relaxed and with the energy required to breastfeed. And frequent feeding will promote ypur milk production.

Keep in mind that newborns babies may experience stress when away from their mothers for too long.

Partners will be doing their part to care for the new family. Burping and changing their baby between feeds and providing skin-to-skin contact as well. Birth companions will also be supporting the mother, supplying her with food and drink, and ensuring that guests are not interfering with her needs to feed the baby, feed herself and rest.

While you give to your baby be sure to indulge yourselves with plenty of lying in bed gazing at the miracle that you have created.

And because this time of year is also a season of miracles, celebrate yours, inside or outside.

Monika describes her birth this way:
I was imagining a short labour but I never imagined a labour that could be so short! She also tells us that her husband Dave now has a new career!

The Birth of Maya

Well, it has been a long time coming, but finally on November 14, Dave took a brave step and transitioned into a new career.  It was a decision he had to make quickly, considering the circumstances.  Our midwives have confirmed it: Dave has earned his honorary degree in Midhusbandry.

On November 14th, at 1:20 in the morning, Maya Belize McLaughlin flew through the barely-filled birthing-pool, like a torpedo fish, into the hands of her father.  The labor happened so quickly that even our midwives didn't make it in time to attend her birth!  From the first surge to birth, labor only lasted 45 minutes.  When we realized that nobody would be there to assist with the birth, Dave called 911 for guidance.  He quickly realized there would be no time to take instructions, dropped the phone, and improvised.

Shortly after Maya was born, however, 6 men in uniform (2 EMS, 2 police men and 2 firemen) showed up to greet her.  Each one wanted to be the hero in his own way, only to find a happy and healthy baby nursing in my arms.  One emergency worker really wanted to use his oxygen tank on our breathing, happily-nursing newborn.  When we politely asked him to leave the baby alone he packed up his equipment and stormed off, citing that he, after all,  "was here to help us."  If only he had arrived 5 minutes sooner he may have had the chance to play the hero, but that title rightly belongs to Dave! (But seriously, we are very grateful to all the emergency personnel that rushed to our side!)

Maya created such a stir that even our neighbour came over to make sure everybody was okay.   Liam, of course, slept through the whole event in the bedroom next door, and was very surprised to find that his baby sister was no longer in mummy's tummy when he awoke.  He asked if this is indeed mummy's baby, noticing that the big belly is now gone.  I assured him that this is mummy's baby, and Liam's baby and daddy's baby - at which time Liam politely pointed out that this is not daddy's baby because daddy still has his big belly!

As you can imagine, my balloon breathing wasn't quite working, and things happened so quickly that I didn't even realize I was supposed to be breathing my baby down... instead of filling my balloon.  I guess most of the labour happened - without my conscious knowledge of it. In my sleep perhaps? 

This was quite a surprise given that Liam's birth lasted 24 hours.  When I was in labour with Liam, Dave had enough time to fill the birthing tub with buckets of boiling water.  Not this time!  He hooked up the hose, and we barely had enough water to reach my belly button when she came swimming out.

Be Patient if Labour Stalls

A 2008 retrospective study published in Obstetrics and Gynecology 2008 looked at differences in outcomes for mothers and babies following a stalled labour. A stalled labour was defined as no cervical change for 2 hours despite adequate uterine contractions.

The majority of women in the study received a Caesarean; 33% birthed vaginally. The authors found an increased risk of postpartum hemorrhage and infection following surgery. There were no differences in the health of the babies.

The authors recommend that prevention of Caesarean be given the highest priority as surgical birth may impact negatively on the new mother's immediate postpartum recovery and on her subsequent birthing experiences.
