2012-2013 PSAS Application - FINAL REMINDER

In order for your family to be considered for tuition assistance for the 2012-13 school year, the PSAS application, including all supporting documentation, must be completed and postmarked no later than December 1, 2011.

In early October all current CJ students received a PSAS application form with instructions for completing the hard copy, downloading the application from cjeagles.org, or submitting an on-line application directly to PSAS. If you do not have this information, please contact Cindy Budde immediately at 461.3740,  x214.

The December 1 deadline allows the tuition assistance committee to determine and announce the assistance awards by early February 2012. Incomplete applications cannot be processed and will be placed on "hold" until all paperwork is received at PSAS.

If you have questions about the PSAS application, please call PSAS at 440.892.4272 or CJ at 461.3740 to speak with one of the following staff members:

Attention Parents of Junior Retreat Participants

If your student is signed up to attend the December junior retreat (view list of participants) and will need to take medication while they are gone, please have your doctor complete an Authorization for School Personnel Administration of Medication Form and return it to the nurse's office by Monday, Nov. 28. You will need one form for each medication that the student will need to take.

Authorization from your doctor is required for all medications, even if they are over-the-counter and all medications are required to be kept by staff. Students are not allowed to carry or self-administer their own medications unless it is an asthma inhaler or epi-pen, in which case an Authorization for Self-Carry Administration of Medication form must be on file with the nurse. Please refer to the Medication Administration during Overnight School Trips Procedure or contact the school nurse at 461.3740  x227 with any questions.

All health forms and procedures are available from the CJ Health Clinic Web pages under the Student Life tab.
