Help Us Help You...and Your Patient.
As you may know, your referral actually begins long before your patient arrives at our hospital. Thus there are a few key points we would like you to remember when making a referral to help ensure a unique and positive experience for everyone.
Properly complete the MidWestVET Patient Referral Form
This is your opportunity to provide a brief case summary to empower us with enough information to get a quick handle on the case. If you need some patient referral forms, please let us know and we will get some to you ASAP. You may also visit our website (www.midwestvetspecialists.com) to print these forms. And for your convenience, you may also fill out the form and submit it online!
Send your complete doctor's notes
Our opportunity to review doctor notes in advance of your client's consultation is instrumental to helping us provide the most comprehensive service. Lists of services generated by various medical records software do not provide an adequate medical history. Indeed as we review your notes, we try to follow your thought processes in our effort to provide the best care for your patient. (Why was the patient presented to you? What did your physical examination identify? What tests did you perform? What medications, including doses, did you prescribe?)
Send the actual print-outs of all diagnostic tests
Many times, a CBC, biochemical panel or UA may be reported as "unremarkable" in medical notes, yet the report may contain important diagnostic information; additionally trends may be seen when the results are reviewed chronologically
E-mail radiographs (or at least send them with your clients)
Images transmitted via e-mail should legally be in DICOM format, and may be sent to info@midwestvetspecialists.com. Alternatively you may send acetate films or CDs of DICOM images with your client, but please make arrangements with your client to ensure they will have the images well in advance of their scheduled appointment
In summary, we request you send ALL medical records well in advance of your patient's scheduled appointment at MidWestVET.
