Greetings CCA Athletes and Parents!
ATHLETIC PHYSICALS
FALL SPORTS

If you are interested in playing a Fall sport, you will be able to get your athletic physical at a reduced rate at San Diego Sports Medicine (regularly $75, CCA students $40 with student ID). Fee will be paid directly to San Diego Sports Medicine; they are extending this discount as a courtesy to CCA athletes.

Please bring your completed and signed front page of Pre-Participation Physical Evaluation & CCA Sports Physical forms.

All new physicals need to be done AFTER June 1st, 2010 to be valid for the 2010-2011 school year.

WHO: Anyone interested in playing a sport in Winter or Spring 2010 - 2011

WHAT: Athletic Clearance Physical Examinations
-- Performed by local physicians volunteering their time
-- We are planning on male and female physicians

WHEN: Thursday, September 23rd from 5:00 - 8:00 p.m.

COST: $25 cash or check made payable to CCA Foundation
** All proceeds will go directly to support CCA Athletics **
