Become a Model Community - Apply for a Grant

The Cook County Department of Public Health (CCDPH), in partnership with the Public Health Institute of Metropolitan Chicago has announced the availability of up to $4 million in grant funding for local efforts to combat obesity in surbuban Cook County.
Successful applicants from local governments, community organizations and schools serving suburban Cook County will receive funding between $25,000 and $200,000 to advance their proposed local initiatives focused on making it easier for individuals and families to make healthier choices.
This program is supported by the Alliance for Healthy & Active Communities, a collaborative effort led by CCDPH, with funds from a federal grant administered by the Centers for Disease Control and Prevention as part of a nationwide effort aimed at addressing the rising rates and burden of obesity in the United States.
