Program Highlight-
In-Home Therapy Services |
In-Home Therapy Services are a structured, consistent, strength-based therapeutic relationship between a licensed clinician and the youth and family for the purpose of treating the youth's behavioral health needs, including improving the family's ability to provide effective support for the youth to promote his/her healthy functioning within the family. Interventions are designed to enhance the family's capacity to improve the youth's functioning in the home and community and may prevent the need for the youth's admission to an inpatient hospital, psychiatric residential treatment facility or other treatment setting. Specific supports through In-Home Therapy include: ~ Enhanced problem-solving, limit-setting, risk management/safety planning, communication to build skills to strengthen the family, advance therapeutic goals, or improve ineffective patterns of interaction ~ Identifying and utilizing community resources ~ Developing and maintaining natural supports for the youth and parent/caregiver(s) in order to promote sustainability for treatment gains ~ Phone contact and consultation are provided as part of the intervention ~ 24/7 telephonic support Referrals Primary care offices, mental health clinicians, schools, case workers, community organizations, faith leaders, parents/caregivers and families can make referrals directly to Family Continuity or through the local Community Support Agencies (CSA). Cost Service rates are payable under the benefit plans through any of the Managed Care Organizations for Mass Health (MBHP, BMC, Beacon Health Strategies, Network Health, or Neighborhood Health Plan). Private pay rates available.

Loving Families Needed |
Family Continuity's Therapeutic Home Care Program is looking for loving: Therapeutic Foster Homes To provide structure & support for children with emotional, developmental, or behavioral needs, with the ultimate goal of reuniting the children with their families. Respite Homes Are also needed for our clients who are living full time with their families, in order to provide a safe place for the child to stay for brief periods of time on a scheduled basis & in times when the child's family is in crisis. As a Foster or Respite Home, you will receive: ~Therapeutic Training ~ ~ A Monetary Stipend ~ ~ Access to our 24-Hour Crisis Hotline ~ Please join us in making a difference for these children & their families. Andrea's Story My name is Andrea and I have a four year old son Sam who attends our local school's special education preschool program. I am a single parent whose job requires me to be at work before Sam goes to school. Luckily, Sam has been going to a family day care home about two blocks from our home. Just as school was ending in June, I started to hear rumors that our town was going to stop providing transportation for children in day care homes. I was told that he would have to be transported to and from my home. I was lost and did not know where to go for answers.
