CRITICAL INCIDENTS & PHONE TREE: PLEASE PRINT!
If you are a training provider who
hires/supervises the lifeguards or swim instructors
you train, it is imperative that you contact SAI following
any Critical Incident at one
of your facilities. A critical incident is:
- An unresponsive victim for whom CPR was
performed and/or an AED was used
- EMS transport involving a confirmed spinal
injury
- Recreational Water Illness (RWI) outbreaks
- UNUSUAL catastrophic events which involve injury
and EMS transport
Below are the emergency contact numbers for
SAI. Please call the number(s), in the
order in which they are listed, until you reach
someone to report the incident.
Our intent is that you make and laminate as
many copies as you need. We recommend that you
also post this in appropriate places at ALL of your
facilities.

AQUATIC INCIDENT DATABASE (AID) LOCATED IN
THE CMS. PLEASE CONTACT SARA POE TO GAIN
ACCESS TO THIS DATABASE AS SOON AS
POSSIBLE.
