    In a recent article, "Treating Chronic Pain: New Knowledge, More Choices" by Christine Whitten and colleagues (published in the [Kaiser] Permanente Journal, Fall 2005/Vol. 9, No. 4), the authors state that multi-modal treatment is becoming more and more important for managing pain conditions. The vicious cycle of pain has been well documented. In this article, several factors that influence this dynamic are listed including: Fear and anxiety, depression, disturbed sleep, changes in the nervous system, guarding in the musculoskeletal system, loss of plasticity in the affected nerves, and muscle atrophy.

    A central point made is that, "we do not yet know the exact combination of pain severity, duration, etiology, and genetic predisposition that leads to chronic pain but one important conclusion is clear: Inadequate pain control increases the likelihood that pain will become chronic." This means that persistent pain problems should be treated early, aggressively, and effectively.

    Some of the multi-modal approaches recommended, in addition to multi-modal drug therapies, are: acupuncture, use of TENS, hypnosis, distraction (such as involvement in engaging activity), and reduction of anxiety and depression. Missing from this list, however, is attention to the trauma that may have created the pain in the first place, the trauma that results from an ongoing pain condition, and the preexisting trauma, such as childhood abuse, neglect, and loss, that may have created a foundational vulnerability which sets the stage for pathological response to future traumatic events.

    My upcoming book, Reversing Chronic Pain, addresses this issue. It's the first book I know of that emphasizes the trauma-pain connection and offers simple, multi-modal approaches that treat the trauma component as well as the pain.

These teleseminars will replace the previously offered 10-week online pain course and are designed to help pain clients and practitioners alike apply the book's principles and techniques, and answer reader's specific questions.
