    New Department of Health & Human Services Initiative to Reduce Hospital Readmissions

    The Community-based Care Transitions Program (CCTP) is part of the Center for Medicare and Medicaid Services' Partnership for Patients, a public-private partnership aimed at cutting preventable errors in hospitals by 40 percent and reducing preventable hospital readmissions by 20 percent over a three-year period.

    CCTP was created through the Affordable Care Act to test models for improving care transitions from the hospital to other settings and reducing readmissions for high-risk Medicare beneficiaries.

    In mid-March, 23 additional program participants - acute care hospitals and their community based organization (CBOs) partners - were announced. This group joins the 7 participants announced last year.

    Up to $500 million in total funding is available from 2011 through 2015 for CCTP to improve transitions of beneficiaries from the inpatient hospital setting to other care settings, to improve quality of care, to reduce readmissions for high risk beneficiaries, and to document measurable savings to the Medicare program.

    CCTP encourages communities to come together and work to improve quality, reduce cost, and improve patient experience.

    New Research Study on Patient Safety and Communication
    HealthGrades


    "Providers who communicate well also do a better job at keeping their patients safe, suggests new research from HealthGrades

    "Researchers found that hospitals with the highest patient ratings in physician and nursing communications have fewer patient safety events, such as surgical inpatient deaths with treatable complications, pressure ulcers and post-operative respiratory failure and sepsis.

    "Conversely, hospitals that perform in the bottom 10 percent for physician communication saw 15 percent more patient safety events, compared to the top 10 percent. The risk in patient safety events was even higher with poor nurse communication.

    "Twenty-seven percent more overall patient safety events occurred in hospitals performing in the bottom 10 percent for nursing communication, compared to the top 10 percentile."
