For the upcoming learning sessions in October I would like to ask each hospital to bring at least two documents you have either developed for this initiative or have changed due to this initiative i.e. policies you have written or changed, parent education sheets etc.  Please bring enough copies to share with the other hospitals in the initiative, tools for everyone's toolbox... 

 

 

Cultural Transformation

 For cultural transformation to occur there has to be a "call to adventure".  Factors that serve as a catalyst for culture change can be grouped into 5 broad categories:

  1. Burning Platform/Significant Threat:  factors might include:  financial crisis, strength of competition, high turnover, mediocre patient satisfaction.
  2. External Restructuring:  factors might include:  mergers, acquisitions, system expansion
  3. New Facilities or Technology:  factors might include:  construction on new buildings that reflect patient needs and wants, implementing new EMR
  4. Passion to be Better/the Best:  factors might include:  opportunity for increased productivity and staff morale, improvement in safety standards or outcome measurements
  5. Visionary Leadership:  factors might include:  action-oriented risk takers, team builders skilled and experienced at creating and managing change.

When you attempt change what category most often motivates the change? Answering "the call to adventure" is an act of courage whether the motivation is driven by passion or desperation because once "the call" is answered; the journey is about to begin.

When Is Primary Cesarean Appropriate: Maternal and Obstetrical Indications

To describe appropriate maternal and obstetrical indications for primary cesarean delivery. The list of potential indications is long. Among all maternal and obstetrical indications, labor dystocia is the most common; multifetal pregnancy and malpresentation are not

infrequent. Maternal indications, including human immunodeficiency virus (with high viral load) and herpes simplex virus (with active lesions), are rare. Preeclampsia alone typically is not an appropriate indication for cesarean delivery. Although the need for a cesarean is absolute for some conditions, such as complete placenta previa or placenta accreta, minimum criteria for a cesarean are variable and subjective for many indications, including dystocia. The subjective diagnosis of labor dystocia provides the best opportunity to prevent the first cesarean.

 

17 alpha-hydroxyprogesterone caproate to prevent prematurity in nulliparas with cervical length less than 30 mm

OBJECTIVE: We sought to evaluate whether 17 alpha-hydroxyprogesterone

caproate (17-OHP) reduces preterm birth (PTB) in nulliparous women with a midtrimester cervical length (CL)30 mm.

CONCLUSION: Weekly 17-OHP does not reduce the frequency of PTB in

nulliparous women with a midtrimester CL30 mm.
