What: Up to three team members - all three primary team members (physician champion, nurse champion, senior hospital administrator) are encouraged to attend. Lunch and materials will be provided at no cost.

This is a test question and is not required to complete the admission form or to submit a record to the database. If your facility would like to track and record this data you may use this field (time is recorded to the nearest hour) and if you do intend to use this field please let me know...
