My name is April Fogleman and I am an Assistant Professor of Nutrition at North Carolina State University.  I am writing again on behalf of my student, Brooke Reimer, to ask for a small portion of your time to help answer a research question about the use of donor human milk in neonatal intensive care units.  

For this project we are examining attitudes, beliefs, and knowledge regarding donor human milk. We would appreciate your participation in this survey - if you are willing to participate, please click on the link below. The survey does not collect personal information.
