McCaffrey What a fantastic call yesterday.  The journey to increasing exclusive human milk use in the NICU continues. No doubt March is a month for becoming familiar with a new data set which will support the efforts you all undertake to increase human milk use. There is a new area of interest introduced which focuses on the type of pump mothers use after discharge from the hospital and the date time at which they start pumping at home. Several centers have reported that there is a lag of days, and sometimes longer, for moms to establish pumping at home. Is this occurring at your facility? If so consider changes you might introduce to eliminate the gap between maternal discharge and initiation of pumping. This may certainly improve pumping volumes at 2 weeks. Is the pump that a mom is using  able to provide proper support for a VLBW newborn infant? If not consider system changes that need to be made to assure that mothers are equipped with optimal pumps.  If you need guidance regarding any of these matters our experts are standing by.  Give Laurie and Polly a call or email them.

Effect of early breast milk expression on milk volume and timing of lactogenesis stage II among mothers of very low birth weight infants: a pilot study

newsObjective: The purpose of this randomized pilot study was to collect preliminary data regarding the feasibility and effects of early initiation of milk expression on the onset of lactogenesis stage II and milk volume in mothers of very low birth weight (VLBW) infants.

Study Design: Twenty women were randomized to initiate milk expression within 60 min (group 1) or 1 to 6 h (group 2) following delivery. Milk volume and timing of lactogenesis stage II was compared between groups using Wilcoxon's rank sum tests.

Result: Group 1 produced statistically significantly more milk than group 2 during the first 7 days (P�0.05) and at week 3 (P�0.01). Group 1 also demonstrated a significantly earlier lactogenesis stage II (P�0.03).

Conclusion: Initiation of milk expression within 1 h following delivery increases milk volume and decreases time to lactogenesis stage II in mothers of VLBW infants.

The North Carolina Breastfeeding Coalition has been awarded a grant to identify 10 businesses in our state that have implemented a lactation support program to highlight in a national data base of breastfeeding friendly employers.  The process will move quickly, and selection needs to be completed by the end of March in order to visit the selected sites prior to May 1.  If you are aware of any employers who have instituted breastfeeding support policies/lactation programs for non-salaried employees please reply to this email with answers to the questions below.

information 1. If you are receiving this email you are on a team that is participating in HM NCCC Phase II - if you think you have received this email in error please contact me.  You will be receiving regular weekly emails (usually on Wednesday) that will share information and resources, keep you updated on your progress, and alert you to coming events.

2. The final version of the data collection form in both PDF and Word versions and the latest draft of the data dictionary have been posted to the HM NCCC Extranet.  (Please contact me if you do not have access to the Extranet as that is essential for retrieving documents and resources).

3. Remember, all teams should start data collection for Phase II as soon as possible.   (Please contact me if you have questions)

4. If you are the data enterer for your team, the new data collection site online and accessible from www.pqcnc.org - select  "Initiatives"  from the right-hand menu, then "Milk (EHM4B)" , then "NCCC Track", then "NCCC Data II'.  Your phase I data login and password will work for phase II as well.   If you are new to the Initiative you will need to register for an account on the data site and I will then be able to give you access. (Please contact me if you have questions or have difficulty accessing the site.)
