Final Newsletter for Phase I

So this is the final newsletter for Phase I. Things to know at this point:

1. The database for Phase I is closing - if you still have data left to enter let me know as soon as possible

2. Final reports will be out soon - watch your email

3. This will (probably) be the last email you receive for Phase I

4. If your team is participating in Phase II you will be receiving a Phase II email soon

5. If you are planning on participating in Phase II remember ALL Teams that are planning on participating must complete a team roster form and return it to me as soon as possible.

6. The final version of the data collection form in both PDF and Word versions and the final version of the data dictionary have been posted to the SIVB Extranet (please contact me if you do not have access to the Extranet).

7. Remember, all teams will start data collection for Phase II on March 1st. New hospitals joining the SIVB initiative for Phase II will complete 2 months of baseline data as well as start data collection on March 1st.
