Phase II Data Collection

 During this month's webinar, teams on the call agreed to finalize the data collection form for Phase II. To view the form, click here.
After much discussion at the November 2nd learning session, the decision was made to complete the form on all patients instead of the previous sampling of 40 patients per month. With our data collection form trimmed down significantly and knowing that sampling is not allowing us to capture the complete picture, data on all patients is important moving forward.
Remember, we will not start Phase II until January so continue date collection and data entry using the current process until that time.

 Contact Amanda if you'd like to 'showcase' your hospital's success on future webinars. As we discussed in the November Learning Session, smaller hospitals have some unique challenges so we'd love to hear more from smaller hospitals on the webinars.
Some things to 'showcase': What are your obstacles? How are you overcoming the obstacles? What successes have you had? What key players are important and how did you get buy-in? What can PQCNC do to further assist in your efforts?
