Two articles of interest...

Recently published in Pediatrics are two articles which may be of interest to all EHM teams.
A scourge in the NICU is necrotizing enterocolitis (NEC). The etiology of NEC is higly specualted upon and is likely due to a combination of several factors. There has been an association established between the use of indocin and postnatal steroids with subsequent development of spontaneous intestinal perforation in premature infants. Sood BG et al report on an association of indomethaicn tocolysis used within 5 days of delivery with the development of necrotizing enterocolitis in the first 15 days of life. The confidence interval is wide (2-20) but the OR is 7. This should at least prompt some discussion with obstetrical colleagues regarding use of antenatal indocin as a prophylactic for preterm labor. A second article from Hauck Fr et al is a meta analysis that provides further evidence that breastfeeding is protective against SIDS, and that the effect is greatest when breastfeeding is exclusive. This is one more strong piece of evidence that support for breastfeeding should be a part of programs aimed at reducing the incidence of SIDS. Click here to read this article.
