Literature Review about feedings and transfusions submitted by Laurie Dunn, MD

"A new topic that is getting attention in the literature is transfusion-related NEC (also known as TRAGI, transfusion-related acute gut injury). One of the questions is whether or not feeding practices surrounding transfusions would decrease the occurrence of this devastating illness. Two new articles this month address this. A retrospective case-control study (El-Dib M, et al. Red blood cell transfusion, feeding and necrotizing enterocolitis in preterm infants, Jnl of Perinatology (2011);31:183-187) compared two eras of practice in their unit in Washington DC. Feeds were held during transfusions only in the latter era; it appeared that withholding feeds was protective. However, another article (Blau J, et al. Transfusion-related acute gut injury:necrotizing enterocolitis in VLBW neonates after packed red blood cell transfusion. J Peds (2011);158:403-409) from Valhalla, NY details their unit's experience, in which TRAGI occurred despite withholding feeds surrounding the transfusion. Perhaps future large-scale studies involving more than one center will help shed more light on this, and will help us understand if modifying feeding practices can help protect against this "TRAGIc" disease."
