Each Newborn Critical Care Center should have standardized the process of getting complete forms for data entry. A new form is started for each admission less than 72 hours old and less than 1500 grams. For the first 14 days the form is completed each shift; the opportunity for each nurse to prompt the oncoming nurse exists and assists in obtaining complete data forms.
What prompts have you developed to assure data collection @ Day 21, Day 28 and at discharge? Email (to metzguer@med.unc.edu) a description of your "system" and if judged "most likely to succeed!" you will earn a prize for your Human Milk Initiative Team. Entries must be received by 5pm on March 9 2011.

Send me your tough questions! Today's submission is about babies who meet the criteria but whose mom's are HIV+ at birth. The answer comes from Laurie Dunn, MD, who indicates we have no exclusion criteria for the initiative when babies are admitted at less than 72 hours old and less than 1500 grams. "I think we had decided to collect info on all <1500 gm babies, even if mother chose not to provide her milk, so I think we should collect info on this baby. Decisions regarding what alternative milk will be used, as well as if modifications to kangaroo care need to be made (not to allow lick and learn, for example) may not be straightforward. But supporting this mother's involvement with her baby will still be very important." As with other mother's not providing milk for their infants, there are a few questions about pumping on the data form which will not be asked of this mother either.
