From the Data Dictionary...
  
dictionaryToday I want to highlight the shift question about supplemental feeding and refer you to the Data Dictionary information. This question will provide data about babies when a supplement is given to help you understand when you have documented maternal or infant indications and when it is given for non-clinical indications.

"If supplemented, are clinical indications documented in the infant's medical record? (Please mark all that apply)
a.    None documented
b.    Maternal indications (illness resulting in separation of infant and mother, maternal medications contraindicated in breastfeeding, known HIV infection, etc.)

Infant Indications:
c.    Asymptomatic hypoglycemia documented by laboratory blood glucose measurement
d.    Weight loss of 8-10%
e.    Hyperbilirubinemia
f.    Other infant indicators"


Data Collection
 
data entry Each maternity center should have standardized the process of getting complete forms for data entry. A new form is started in L&D either from each term delivery (many centers are collecting information on each delivery) or from the first term admission or delivery on day shift (excluding scheduled operative births) and the first term admission or delivery on night shift. This is an acceptable method of random audits and no other factors should be considered in starting data forms.

What if...?
 
question marks Send me your tough questions! Today's submission is about babies who stay in the hospital after their mother's are discharged. These babies most likely have medical concerns that prevent their discharge and the data collection tool cannot be adequately completed without the mother in house for shift data. Additionally, the discharge form cannot be completed. Therefore these mother baby dyads cannot be entered into the web data base.
