Last week in this email we shared that a critical element in determining whether changes your facility will make to increase exclusivity are effective requires baseline data as well as ongoing measurement. While some of your data sheets may be incomplete and unable to be entered in the data system, we would still like the opportunity to review that data. 


Please copy and send us  your data collection forms as soon as possible and before January 5, 2011:

How do I enter data?

Please review and follow the instructions in the email sent by Keith on December 20th with subject "PQCNC Milk Well Baby DATA is now LIVE!" Email me if you do not know your facility code.





How do I create a patient ID?

information Each patient ID on the data entry site is 4 digits in length. The first digit is your perinatal region. Then the first patient is 001, second patient 002, etc. Therefore, first 3 patients in perinatal region 3 are 3001, 3002 and 3003. Those same 3 patients in perinatal region 6 are 6001, 6002 and 6003. Together with your facility code this creates a unique ID for every patient.
