I will be reviewing a recent article once a month that pertains to the Support for Birth project. Click here to read my review of a July 2010 Green Journal paper about labor induction and the risk of cesarean delivery. As your team begins to identify what processes in your hospital to focus on to increase the rate of vaginal births in the Nulliparous, Term, Singleton, Vertex (NTSV) cohort, it may be helpful to learn about high-impact processes from other settings. In this month's article, the major modifiable process which contributed to the cesarean section rate in a large population of NTSV women at a regional hospital was induction of labor (IOL). This paper is one of many which document this association. IOL is a process with many steps. If your hospital offers IOL, your baseline data may show an association with increased rates of CS also. Factors to consider might include: - Which women are being induced?
- What are the indications for induction?
- Are women with unfavorable cervical exams and no medical indication being delayed or having cervical ripening done?
- How is cervical ripening done at your hospital? Is there a standard technique? Is that technique evidence-based?
- What is the pitocin protocol at your hospital? To what extent is it followed?
- How does your hospital define "failure to progress"?
In future reviews of journal articles, I'll try to address some of these issues.

This year CIMS is holding its annual meeting in Chapel Hill in conjunction with the annual Breastfeeding and Feminisn symposium sponsored by UNC Greensboro and the Carolina Global Breastfeeding Institute.
Called "Reframing Birth and Breastfeeding: Moving Forward," the conference will be held at the Sheraton in Chapel Hill on March 11-12.
There will also be a pre-conference workshop called "The Art of Mother-Friendly Labor Support for Nurses" on March 10. Click here to access the conference's webpage.
