DATA!

data entry We have received lots of October baseline data.  If your team has not sent yours, please send it as soon as possible.

Here are a few pointers:

1. If the patient is NOT an NTSV, you do not need to complete a form (but you're welcome to if that's easier; that data will not be entered into the database).
2. If the patient is NTSV, complete a form even if she is admitted for c-section: this is a "short" form.
3. Collect data on all NTSV patients - no skipping!
4. The "cervical exam at admission" section has some areas of confusion.

  • Report a single whole number for the cervical dilation (no half-centimeters or ranges).  If the admission note states the cervix is 1.5, report that as 1cm.  If the admission note states the cervix is 1-2cm, report that as 1cm.  If the admission note reports the cervix is "fingertip" report that as 0cm.
  • The "position" field in this section refers to the position of the cervix, not the baby.  The three options are: posterior, mid and anterior.
  • The options for "station" are -3 to +3.

5. Please do not provide the patient's actual medical record number under "chart number."  Your team may use any numbering system that works best for your site.  It is important to identify a system so that multiple records do not end up having the same chart number.  Feel free to contact me for suggestions abuot how to number the charts.
 
For hospitals that are sampling:

·    Collect data on all patients at least until you've reached 40 "long" forms. This means your total number of forms will be more than 40, because it will include the "short" forms that were filled out until your monthly sample was complete.
·    If you completed October baseline data, begin collecting data on November 15.  If you already started on November 1, that is also fine.
·    If you did not complete October baseline data, collect extra data this month (80 cases).
·    You must submit a minimum sample that includes 40 long forms and as many short forms as it took to reach 40 long forms.  You are welcome to submit a larger sample.

On the Extranet

SIVB Extranet At this time, the Extranet is where you can find data collection materials.  As this initiative enters the active phase in January, we will post Powerpoint presentations, relevant articles and links to resources. 

To access the Extranet, you must be a SIVB team member and have an account on www.pqcnc.org. You then log in and click on "Initiatives" on the right-side menu, then click "Support for Birth" and you will see SIVB Extranet as an option.

So:
·    If you can currently see the link when you are logged into www.pqcnc.org there's nothing you need do - you have access.
·    If you have registered on PQCNC.org and do not see the link please email Keith and tell him your login ID and he will give you access.
·    If you have not yet registered on PQCNC.org you can do so by visiting http://www.pqcnc.org and clicking on 'Login' on the top menu bar and selecting 'Create new account'  - after you create your account please email Keith with your login ID and he will give you access.

Be sure you are using the current version of the data collection form (it includes date AND time of admission at the top, and date and time of delivery at the end).  The data collection form can be accessed on the SIVB Extranet page.
