Webinar kicks off Baseline Data Collection

data entry Welcome to all the teams who have joined the PQCNC 2010-11 Initiative to support the mom of the VLBW infant to provide milk for her baby for the first 28 days. At each hospital teams are convening around the first task of collecting baseline data. Click here to print the tool for data collection and click here for the instructions for data collection. We will have a meeting (phone only) on October 13 at 10 am to discuss data collection; please have at least one member of the team participate. The call-in number is 919-962-2730. Encourage your staff to just document what's going on during the baseline data collection and resist the urge to change practices right now. Your team will leave the January learning session ready to address practices that support moms and babies for exclusive breastfeeding.

conference callAll baseline data is collected on paper forms. When the form is complete place the infant hospital stamp/sticker on the form. In December we will be coaching data entry personnel to use the program on the PQCNC website. Each patient will be assigned a unique ID and no HPI will be entered into the program. Now is the time to begin your planning for data entry; I will ask you to share your preliminary plans with us on our second phone meeting about data collection. We will have a meeting (phone only) on October 28 at 3 pm to discuss data collection; please have at least one member of the team participate. The call-in number is 919-962-2730.

Timeline and Dates to Remember

calendar October and November: baseline data collection on all infants admitted who are less than 1500 gms and less than or equal to 5 days old.

December: Data entry

January 13th  in Winston Salem: Each team will attend the learning sessions as a group. Details about the day will be provided later in October; please begin thinking about which team members might be part of the "travel team."  We suggest that the home team be as inclusive and as large as you can manage; the travel team will be limited to 6 individuals representing key stakeholder groups.

February 8th, March 8th, and April 12th: Monthly webinars from 2-4 pm to provide education and coaching during the action period for improvement activities.
