Baseline data collection is underway!

data entry ·    Hospitals should be in the process of collecting baseline data as of October 1.  I apologize again for the confusion with the data collection form last week.  The correct version of the form is available here.  Please destroy all other versions of this form, and I will plant trees in the name of this project to make up for the wasted paper.  Click here for the Data Definitions document, and here for the Sampling Plan for large hospitals.  If you have trouble accessing any of these links, contact Kate for assistance.

·     A data collection form should be filled out on every nulliparous (no deliveries past 20 weeks) woman at >/=37 weeks with a singleton, vertex fetus.  Unfortunately, you cannot use "gravida 1" as a way to identify your population, because G2's and beyond may not have given birth beyond 20 weeks in the past.
 
·     Follow the instructions on the form carefully: if a patient has a placenta previa, for example, you do not have to complete the entire form but you do need to submit that information, so keep that form for when it comes time to enter data.  If you are sampling, your 40 charts should include only those who are eligible to have the entire form completed (the others are extra!).

·     Chart number: Each term should determine a strategy for how to number the charts you will submit.  You may simply number them 1-40, or you may use the chart number as a way to code your data by the provider and/or nurse caring for the patient, using initials, letters or a numerical code.  Please do not use the actual medical record number as the chart number.

January 2011 learning labs

learning session The one-day learning session to officially launch this initiative will be held twice:  Tuesday, January 11 in Winston Salem and Tuesday, January 25 in Raleigh.  Each team needs to attend one session as a group.  There is no charge to attend, and CME Category 1 credits (good for MDs, RNs, PAs, CNMs, and NPs) will be provided.  All key team members (physician champion, nurse champion, senior hospital adminstrator) are expected to attend, and each team is welcome to bring up to 6 team members.  Teams are encouraged to include personnel from quality/performance improvement, as well as staff nurses, patient advocates, educators and others.  Registration will be available on the PQCNC website later this month.  Please determine which date your team will attend and make sure team members mark their calendars.  It is the expectation that physician champions will attend this session.  During last year's initiative, it became clear that teams whose physician champion was actively engaged from the start were able to implement more changes.
 
If you do not have a complete team roster at this time, do not worry.  However, now is the time to assemble the stakeholders your team will need in order to make needed changes to improve the vaginal birth rate.  Please contact us for assistance in this process.  Dr. Chescheir, the clinical leader of this initiative, is available to reach out to physician champions who would like to learn more about the project.

PQCNC has set a goal of increasing the rate of vaginal birth by 25% across the collaborative.  Each team will set a site-specific goal at the learning session in January using your baseline data as a guide.
