Support for Birth data collection begins October 1!

data entry There are 3 key documents in this email which you will need in order to be ready to start collecting data on October 1, the revised data collection form, the data definitions document, and the sampling plan.  I have also updated the slides from the September 21 webinar and included them as well.  If you did not attend the webinar, please review all four documents carefully (form, definitions, sampling strategy, slides) and contact us with any questions.  We are excited to get the Support for Birth initiative started and to begin to explore ways we can address the climbing c-section rate as a statewide collaborative.

1.  Data collection form - this is a 2-page form that should be completed for every nulliparous patient delivering on your unit.  Depending on certain factors, you may not need to fill out the entire form for each patient.  Please follow the instructions carefully.  For some patients, you will need to enter data only for the first one or two questions, while for others you will need to complete the entire form.  Required questions are marked with an asterisk (*), but we hope you will make every effort to obtain complete information.

This form has been modified since the September 21 webinar.  You will know you are using the correct form because it says "PQCNC SIVB Data Collection Worksheet - Final" instead of "Draft."  Please destroy all copies of the draft form to avoid confusion.

As a reminder, the case should be entered in the month in which the patient was admitted, not delivered.  If you have any questions about how to use this form or about any question on the form, please call or email me anytime.

2. Data collection definitions - Please review this document and make sure all staff who will be collecting data are familiar with it.  It is important that we all use the same definitions so that the data will be meaningful.  If anything is unclear, don't guess - call or email us anytime!  

3.  Sampling strategy - We have a sampling plan!  This means that no hospital will have to submit more than 40 forms per month.  Review this document carefully to see if the sampling plan will apply at your hospital.  If you collect 40 forms by the end of the second week of October, you may qualify for sampling.  The count of 40 cases should be based on patients for whom the entire form was completed, not patients who only qualified for the first one or two questions.
