QH AYSO is going the way of the Online Application!
 
Online Applications can be completed at  www.eayso.org and are required for the 2012 Season for both players and volunteers.  There will be no pre-printed forms for players or volunteers returning from 2011 nor will there be any blank forms to fill out.  Only online applications will be accepted this year.


BEFORE YOU BEGIN: Make sure you have the most updated version of Adobe Reader installed on your computer and that you use either Internet Explorer or Firefox as your web browser. Mac Users must use Firefox.  


Here are some useful tips to help with the online registration including a picture of the eAYSO home page below for reference.


eAYSO Front Page

Past eAYSO Users:  If you have used the eAYSO system in the past but have forgotten your Login E-mail or Password, then click on either "Forgot Login E-Mail?" or "Forgot Password?" and follow the directions on subsequent pages.  Your Login E-mail and/or Password will be provided.  If that still does not work, and you are sure you do not have the cap lock button on, call the eAYSO help desk at (866) 588-2976.

New eAYSO Users with New Players:  On the eAYSO home page, under the Login, click on the button that says "My child is new to AYSO and wants to play soccer" and follow the steps on subsequent pages - making sure to SCROLL DOWN as many of the data screens extend below the bottom of your monitor.  When printing, please do not use the print function under "File" but rather use the printer icon with the flashing red arrow pointing at it (see picture below).

New eAYSO Users with Players that have played in QH AYSO or any other AYSO Region Although your child/children may have played in AYSO for years, if you have never used the eAYSO system, your child/children's information will not be there - you will need to input all of your child/children's information the first time per these directions:  From the eAYSO home page, under the Login, you will need to click on the button that says "My child has played in AYSO but we have never used eAYSO" and follow the steps on subsequent pages - making sure to SCROLL DOWN as many of the data screens extend below the bottom of your monitor.  When printing, please do not use the print function under "File" but rather use the printer icon with the flashing red arrow pointing at it (see picture below).

If you are unable to complete the online application before arriving at a registration, there will be a very limited number of laptop computers and a printer available so you may do so before proceeding through the registration process.  Please save yourself time at registration - complete and print the application at home, at your convenience. 

 

Players: Print out ONE copy of each player's form on a single sheet of plain white paper (doubled-sided), sign and date page 1, initial page 2, and bring it to one of our registration dates listed on the reverse. Remember, proof of age, i.e. birth certificate, is required for all children that did not play in Region 638 in 2011. In addition, if a player did not play in Region 638 in 2011 and will be eight (8) years of age or older before August 1, 2012 they will need to be present for a skills test in order for registration to be completed  

 

Volunteers: Print out TWO copies, each on a single sheet of plain white paper (doubled-sided), sign and date it, and bring it to one of our registration dates listed on the reverse along with a valid ID.

 

 

HOW DO I PRINT DOUBLE-SIDED? 

 

If your printer does not automatically print double-sided, you will need to print page one, reinsert the paper into the printer and then print page 2

Why are we doing this? 

 

The Online Application was designed to help streamline the registration process and has the following advantages: 

  • It takes an average of 4 minutes for our Registrar or CVPA to enter all the required information for one player or volunteer handwritten on a paper application. It will take only 30 seconds to process an application completed online.
  • Required personal identifying information (SSN, DL #, etc.) is encrypted on entry and much more secure than using paper forms. No more hassles for Registrars or CVPAs with incomplete forms!
  • Parents/volunteers only have to do this once. If they choose to save a user name and password, next season it will take them less than 1 minute to sign up again!
  • Parents/volunteers can print the needed copies of their own forms on the spot. Special software is no longer needed to print.  
  • No handwriting to decipher! Fewer data errors! All info is up to date! Registrars need only use a few mouse clicks to accept a player or volunteer who has signed up online. Little or no typing!
