Statin Associated Myopathy and Exercise

Not only can statin drugs cause myopathy ( muscle disease ) of varying degrees but this myopathy is greatly exacerbated by exercise. The effect of exercise is such that most experienced athletes will not use statins for fear of predisposition to muscle side effects of all kinds. 

 

The severity of the muscle side effects from statins vary widely from relatively benign CPK elevation without symptoms, to muscle tenderness, aching, soreness and pain, and rarely, even to death from rhabdomyolysis.

Forgetfulness, Disorientation, Confusion and Statins

The advent of the stronger statin drugs in the past decade has contributed to a flood of reports of impaired cognition. These complaints range from transient global amnesia, to aggravation of pre-existing senility to forgetfulness, disorientation and confusion.

The mechanism of this side effect is clearly rooted in the biosynthesis of cholesterol and fundamental to neurophysiologic mechanisms. Only in the past several years have we learned the importance of cholesterol in brain function.

Imagine, cholesterol, the same cholesterol we have been led by the pharmaceutical industry to be public health enemy number one, now proven to be absolutely vital in the formation and function of the trillions of synapses in our brains.

Causes of Statin Side Effects

I often refer to the mevalonate pathway ( also known as the HMG-CoA reductase pathway ) as all statins are reductase inhibitors and therefore exert their effect at the very beginning of the mevalonate pathway, the location of this key reductase step.

When statin drug manufacturers make the claim of reduction of cholesterol synthesis, it is based upon inhibition of the mevalonate pathway, shared by the other major elements such as CoQ10 and dolichols.

This diagram helps to explain the mevalonate pathway and what inhibition by statins can lead to.
