Members Can Now Order ID Cards & Download Authorization Forms
 
Several new features have been added to the Principal web site to access tools and resources at their convenience. 
 
These are available to both members and their dependents.    
 
You can download a handout describing these features using the link on the right.  Please distribute or email this information to your employees.
Order an ID Card 
 
Members and dependents now have the ability to order replacement ID cards from the Principle web site.  
 
Steps to Order an ID Card
  • Go to: www.principal.com
  • Select "Personal" as the login type from the drop down menu
  • Enter Username and Password
  • Click on "Member Services" (upper left hand side of screen)
  • Click on "Access Your ID Card"

The cards will be mailed to the primary mailing address on file.

Authorization for Disclosure of Protected Health Information (PHI)
 
Principal must protect the confidentiality of a participant's PHI as stated in its Notice of Privacy Practices for Health Information.  This means they can not release PHI to you or a participant's family member without an authorization signed by the participant.  
 
Members and their dependents can now access this form in the Forms section under the Member Services Tab on the Principal web site.

Before you can help an employee resolve a claim, you must have their signed Authorization for Disclosure of Protected Health Information.
