"Case Managers: Harness Your Dedication, Motivation and Caring Hearts"

The recent CMSA annual conference made me more certain than ever that as a group, case managers are dedicated, motivated and caring professionals. It was wonderful to see the level of interest and enthusiasm so many case managers have for their profession and their fellow case managers. Now, if we could only harness that energy and make the difference we all so desperately want to make for patients and the quality of care they receive.

Catherine M. Mullahy, BSN, CRRN, CCMWhat will it take? Well, I've said it before and I still stand by my belief that case managers need to invest in themselves on multiple levels. First, case managers need to continue to participate in continuing education to stay abreast of the many changing dynamics affecting healthcare in America today - healthcare reform, our nation's growing multiculturalism, our aging population, longer life spans and increasing incidences of age-related medical conditions, new medical technologies and pharmaceuticals, and growing rates of diabetes, obesity, etc.

Secondly, case managers must adopt a more assertive, stand-up-for-themselves attitude. Case managers play a vital role in our healthcare delivery system. They are critical to the quality of patient care provided and to patient outcomes. They contribute to a healthcare provider becoming a Center of Excellence and on a less evidence-based basis, case managers contribute to the word of mouth reputation a healthcare organization earns from patients it has treated and their family members.

It is true that, by nature of the setting in which case managers work -- their employers and specific job descriptions -- the patient advocacy role of case managers can be constrained. Still, at the core, case managers are patient advocates. Regardless of their role or setting, every case manager can make a difference by communicating more and more effectively, by sharing ideas and observations regarding the patients in their charge, and by documenting their role and successes on behalf of their patients and their employers.

Lastly, there is a matter of commitment or should I say recommitment. Within every case manager there beats a caring heart. It's the same heart that leads an individual to become a nurse and a case manager. It's the same heart that keeps case managers showing up every day despite the many challenges plaguing our healthcare system today which, in turn, impede their ability to care for patients and perform their role the way they know it should be performed.

The CMSA conference was an affirmation of what I already knew. Case managers want to be the best they can. They want their caring hearts to show when they are caring for patients. They want to be heard because they know they have great ideas and wisdom to share with their colleagues, their employers and the decision-makers shaping the future of healthcare in our country.

Coming off the real high we all experienced at the CMSA conference, I would encourage each and every case manager and case management department supervisors to start afresh with new initiatives. These initiatives should encompass and emphasize continuing education for case managers. They should empower case managers to have a voice in how case management is provided, and they should afford case managers the opportunity to recommit their caring hearts

 

Best in Class Case Management Program Receives Case in Point Platinum Award

On hand to see Mullahy & Associates and the other winners receive their awards were representatives from hospitals, health systems, managed care organizations, provider organizations and other leading institutions. Suzanne Mintz, president and CEO of the National Family Caregivers Association, gave the keynote presentation.

Catherine M. Mullahy, RN, BS, CRRN, CCM, president of Mullahy & Associates, LLC,  (www.mullahyassociates.com,Huntington, NY), recently presented a pre-conference session titled, "Becoming a Manager in Case Management - Gaining the Inside Track," at the Case Management Society of America's (CMSA) 21st Annual Conference held at the Henry B. Gonzalez Convention Center in San Antonio, Texas. Additionally, she was a keynote speaker, whose address, "Recommitting the Caring Heart of the Case Manager for Better Patient Care and Advocacy" received a standing ovation from the over 2,000 in attendance.

Did You Know...?

Not One State Received an "A" in Patient Satisfaction Scores

Based on the Hospital Consumer Assessment of Health Plans Survey (HCAHPS), there was not one state that scored an "A," "A-"or even "B+". The highest score received was a "B" which the states of South Dakota (84.1) and Wisconsin (83.5) received. The largest number of states score in the "B-" range, with the rest scoring "C+" or "C" and the lowest score of "C-" going to the District of Columbia. Based on the HCAHPS survey, the average score which patients gave American hospital was a "B-." Patients rated hospital against eight criteria including: communications with doctors, communications with nurses, responsiveness of hospital staff, pain management, communications about medicines, discharge information, cleanliness of the hospital environment, and how quiet the hospital environment was.

The take-away from this survey is that patients in America are just "satisfied" with the care they receive and that improvement is warranted. Here's another opportunity for case managers to engage providers on the importance of adopting new patient advocacy initiatives that demonstrate to patient that hospitals do care, do want to make their healthcare experience better and are willing to listen and better respond to their needs.

"Apply Leading-Edge Solutions to Achieve More Consistently High Quality Case Management Practices"

In many large private and public sector organizations, case management is a decentralized function which multi-city and state operations. Within these entities, there are disparate systems even when the entire organization is expected to adhere to standard operating procedures. In these instances, and despite the best efforts of case management department heads, it is difficult to maintain consistently high quality practices. One way to help address this problem is to apply a robust case management software program.

There are many case management software solutions to choose from, however, any selection should be governed by the presence of certain features. In particular, look for a case management solution which is:

 

-Web-based enabling case managers to access its tools and information from any location on a 365/24/7 basis;

 

-Transparent affording department heads the ability to better control case management assignments, manage compliance reporting, view medical histories and track cases easily;

 

-Integrated so that case managers in remote locations can be most productive due to their ability to access evidence-based guidelines, disease codes and treatment codes online, and use the system's tools to create and process more accurate documents;

 

-A centralized database of case-related information (e.g., medical records, reports, notes, claims data, etc.) which supervisors can retrieve to review cases, determine work schedules and assignments; and

 

-Comprehensive compliance tools for meeting audit standards and HIPAA requirements.

 

-High-risk patient utilities that: help identify patients; facilitate health assessments including disease-specific assessments and related care plan options; provide automated alerts to case management team members when certain critical events occur and/or actions must be taken; present a full view of the patient's medical history and requirements; generates timely, meaningful reports; and tracks the actions of each member of the patient's medical care team.

 

When considering a case management solution, case management departments should be guided by a reputable provider of healthcare management solutions. Professional associations such as the Case Management Society of America can provide lists of qualified vendors. In addition, asking other organizations that are comparable to your organization by category and size what software solutions they use and what their experience has been can also help steer your organization in the right direction.
