Global Cost Pressures Lead to an Increased Need for Total Cost of Ownership Practices

Increasing cost pressures in the U.S. healthcare industry are well documented, with healthcare as a percentage of GDP rising from approximately 16 percent in 2007 on towards a projected 18 percent by 2015.[1]  Feeling the effects in the clinical laboratory market, a recent broad survey of U.S. laboratory managers and administrators indicated that the respondents were concerned about the rising costs of healthcare, a reduced labor force and general unease over healthcare reform legislation[2]

 

Mirroring these trends, the World Bank in the EU is predicting an increase in healthcare spending from 8 percent of GDP in 2000 to 14 percent in 2030.[3]  So regardless of the healthcare system philosophical debates, the costing trends are similar and heading toward something unsustainable.

 

These macroeconomic challenges are certain and the impact is being felt in laboratories that we visit.  At Nexus we believe that the tools used to combat these challenges are the same throughput the world:  continuous operational improvement, measuring and monitoring techniques, and using accounting tools to control, gauge progress, and help make the right decisions.

 

One of these tools, called Total Cost of Operation, is being used more frequently.  Total Cost of Operation (TCO) is a methodology for determining, as the name implies, how much a certain method costs to operate.  TCO uses activity-based costing (ABC) techniques, which consider all the manual activities of the method, as well as capital and consumable costs.  This ABC approach, popular in manufacturing and other industries for many years, has been applied effectively in healthcare diagnostics and offers a way to better monitor and understand the actual costs of operation.

 

The best way to understand TCO is through an example.  In 1Q11 Nexus visited a U.S. hospital to better understand the costs involved in transporting AP specimens.  There were many containers used to transport specimens, ranging from simple plastic containers to bottles that were pre-filled with formalin.  In all there were over 12 different containers used in the process.  More...

Tools of the Trade: The "Time Study"

 

At Nexus we are constantly striving to better understand the laboratory's environment, needs and processes.  This information is used for both the lab operation (to make improvements) and the company that sells products to the lab (to understand the voice of the customer).  To better understand the operation, one of the most frequently used tools is the "time study".  A cornerstone of the discovery process, time studies reveal many things about the customer, not the least of which is the workflow and processes.

 

The only way to perform a time study is to watch in detail every aspect of the operation that is of interest.  If an instrument throughput and labor times are needed, then this might require an 8-hour study of the instrument with each and every detail observed and timed.  The steps in the process (called elements) are catalogued and timed.

 

Sometimes the process can be digitally recorded and studied later, but more often than not, video is not adequate on its own.

 

Nexus generates many reports and analysis.  Although some conclusions can be enhanced with interview data, the cornerstone of all Nexus conclusions are objective data acquired through actual on-site observation such as time studies.
