Poor Infection Control Procedures Identified through Continuous Improvement Techniques

A federal study released last week finds that same day surgery centers may be a breeding ground for nosocomial infections due to their lax approach to infection control.  The study, appearing in last weeks' Journal of the American Medical Association, depicts ambulatory surgical center (ASC) healthcare workers as inconsistent in such things as the handling of blood glucose monitors and the cleaning of equipment[i].  In addition, it was not uncommon for workers to use single dose medication vials for multiple patients.

These discoveries were made by The Centers for Medicaid and Medicare Services (CMS) as they piloted a "new audit tool".  This approach involved physically following patients and observing interactions with staff and the overall workflow involved in "processing" the patient through the ASC.  Interestingly, this "new audit tool" pioneered by the CMS is actually a very old tool, and one that is a cornerstone to the continuous improvement methodologies...

Tools of the Trade

 

Measuring laboratory performance can be difficult.  At Nexus we have found that it is best to have a "metrics dashboard" to regularly compare performance over time, thereby isolating problem areas.  A common metric in the laboratory is cardiac turnaround times, but there are many other metrics that should become regularly measured to include: asset ratios (how well is the lab using capital equipment and other assets); performance ratios (objective performance calculations); and TAT ratios (specific turnaround time measurements that can help identify opportunities).
