President's Page: Accountable Care Organizations (ACO) - or Lack of Accountability in a Disorganized Organization
By: Nathan Laufer, MD
| | In the August 2011 issue of Round-up I wrote an article reviewing the current status and rules of ACO's. There has been much discussion in the medical

literature and much angst about this new type of healthcare delivery system. There is also much confusion and speculation.
As of this article, the final rules have not yet been written. It is the development of the ACO that has caused so many physicians to hide under the hospital umbrella, lest they be left out and risk losing their patients. My personal opinion is that this type of organization will not survive. There is already much pushback from organized medicine, payers, and patients. This system is essentially a modified form of capitation where under-delivery of healthcare is the incentive. Read more...

A Review of The Current Status of Accountable Care Organizations
By: Nathan Laufer, MD
| | |
After many years of dwindling government funding for testing new approaches to healthcare delivery, a key component of the Patient Protection and Affordable Care Act (ACA) reverses that slide. The law allocates $10 billion over a decade to a new Center for Medicare and Medicaid Innovation, which will test other new Medicare payment models, including partial capitation. But Congress also promoted its own preferred delivery model by launching a "shared savings program" involving ACOs. Underscoring its interest in rapid change, Congress made ACOs a
permanent program (rather than a pilot project) with a start date of
January 1, 2012. Participation in an ACO, by both providers and patients, will be voluntary. Read more...

From the CEO: A Call to End Apathy
By: Daniel F. Mitten, CEO/Executive Director
The biggest obstacles to healthcare are not reimbursement, regulations or

rising medical costs, it is simply one thing - apathy. There is no bigger threat to medicine than the indifference of the most influential, important factor in medicine - the physician!
Over the years issues have and continue to plague physicians; however, the overall feeling by the majority is one of unresponsiveness and that one doctor cannot make a difference. But that is not true!
A group is a collection of individuals, and it begins with you. You count, you matter, you have opinions, thoughts, ideas and a voice that can challenge the status quo and are a part of a physician movement that can adapt and possibly direct a new course for healthcare. Read more...

Six Month Check-in: Act Now for the Version 5010 Transition
| | |
The Version 5010 transition is less than six months away for all HIPAA covered entities. This means that to submit transactions electronically, all covered entities must upgrade from Version 4010/4010A to Version 5010. Version 5010, unlike Version 4010, accommodates the new ICD-10 code sets, and is a required preliminary step for the use of the new ICD-10 medical code sets.
Before the compliance deadline of January 1, 2012, you should conduct internal and external transactions within your organizations and with your billing partners - including payers, vendors, clearinghouses and providers. External testing should take place now in order to make sure that you are able to send and receive compliant transactions effectively. Testing now will help identify any potential issues that may arise, and allow the necessary time to address them.
The CMS ICD-10 Web site has resources to support providers, payers and vendors as they make the transition to Version 5010 and ICD-10. Please visit www.cms.gov/ICD10 for the latest news and resources to help you prepare.

Several states could end up paying millions of dollars in payments under the Medicaid portion of the meaningful use program to healthcare providers who are ineligible for the incentives, according to a report by HHS' Office of Inspector General, The Hill's Healthwatch reports.
Under the 2009 federal economic stimulus package, healthcare providers who demonstrate meaningful use of certified electronic health record systems can qualify for Medicare and Medicaid incentive payments. The OIG report looked at 13 states that had approved plans for the Medicaid EHR incentive program by Jan. 14. Read More...

Understanding Bundled Payments
| |
Although 2011 may go down in the history books as the year of the accountable care organization, there's also been a tremendous amount of payment innovation on a different level - aligning incentives and optimizing the delivery of "episodes of care." Bundled (sometimes referred to as episode-based) payment structures are a means to that end. A bundled payment can be thought of as a budget. For a given episode of care, a group of "at-risk" providers agrees to work together to ensure that care is coordinated and that the total cost of an episode is within the budget. Read More...
