Do Not Resuscitate Orders; Let's Prolong Life, but Not Prolong Death

By: Nathan Laufer, MD

Dr. Nathan Laufer
MCMS President

I frequently come across hospitalized patients who were labeled as a "no code" or "do not resuscitate" (DNR). Some of these designations are a surprise since I, as the attending physician, may have just completed a procedure on the patient, and assumed they were a full code. Nurses or residents are now having these discussions without fully knowing the patients. Code status has now become part of the hospital admission checklist. When probed, patients may have the understanding that if they agree to a full code, they will be kept on artificial life support for the rest of their days and therefore refuse to be coded. However, when asked about a simple shock for ventricular tachycardia that may allow them to leave hospital, more will readily agree. Patients may not realize that when they undergo surgical procedures they are kept alive by artificial means.

The Centers for Medicare and Medicaid Services (CMS) announced it would be more flexible in allowing doctors to phase in electronic prescribing technology.

Under current rules, physicians who use a qualified e-prescribing system are eligible for an additional 1% in Medicare Part B payments in 2011 and 2012, and a 0.5% increase in 2013.

Providers who fail to complete at least 10 paperless prescriptions using a qualified e-prescribing system between Jan. 1 and June 30, 2011 will receive a 1% cut in Medicare reimbursements in 2012, a 1.5% cut in 2013, and a 2% cut in 2014. Read More...

Medical Liability Reform Bill Continues to Advance

An essential component of health system reform is repairing the nation's costly litigation system. The HEALTH Act, which includes time-tested liability reforms, recently took a big step toward a vote in the U.S. House of Representatives but it continues to face challenges. Read More...

The Maricopa Integrated Health System has closed a deal with the University of Arizona College of Medicine in Phoenix to become the school's primary training, teaching and research hospital.

Officials hope the partnership will attract physicians, especially primary-care doctors, to the Valley and help halt the growing shortage of doctors in the region and the state.
