Physician deadline for Medicare PECOS enrollment moved up to July
CMS issued an interim final rule making a key compliance date six months earlier.

Physician practices that are having problems with Medicare's enrollment system say their headaches are sure to increase if the Obama administration sticks with its new July deadline for most physicians to be signed up with PECOS, the Provider, Enrollment, Chain and Ownership System.

The new enrollment compliance date for doctors who order or refer most types of covered Medicare services and items is six months sooner than expected. Physicians have been operating under the assumption that they had until Jan. 3, 2011. The previous April 5 deadline had been postponed in mid-February, at the behest of the American Medical Association and other doctor organizations.

Lack of Adequate Pay Reduces Effectiveness of Medical Home
The historic healthcare reform legislation that Congress passed in March makes 19 references to the term "medical home," and for good reason.

Reformers are betting that the medical home, with its physician-led team delivering and coordinating holistic patient care in the exam room and online, is just what the country needs to improve quality and lower costs. At the same time, giving primary care physicians extra pay for operating medical homes - and operating them well - promises to deliver these clinicians from fee-for-service medicine and its piecemeal, perverse, and punishing incentives.

Creating such nirvanas, however, is easier said than done, especially when physicians attempt to reform their practices in an unreformed system with inadequate compensation, according to a set of 8 articles in a special supplement of the Annals of Family Medicine, released today, which evaluate a 2-year medical-home demonstration project.

Working "feverishly," the 36 participating family practices registered modest improvements in quality-of-care measures but backslid in terms of how patients rated them. The demonstration project is the first of its kind on a national scale.

Largest Ever Nurses' Strike Could Be Sign of Future
Unrest
A 1-day strike of 12,000 nurses in Minneapolis and St. Paul, Minnesota - said to be the largest in US history - ended as planned this morning, but experts expect more labor unrest to roil the hospital industry.

The nurses on strike from14 hospitals in the Twin Cities belong to the Minnesota Nurses Association (MNA), an affiliate of an ambitious new "super union" with 155,000 members called National Nurses United (NNU). The NNU advocates nationwide adoption of nurse-to-patient staffing ratios that it deems low enough to ensure good patient care and prevent nurse burnout.

Staffing levels figure heavily into continuing negotiations between the Minnesota nurses and their hospitals over a new labor contract. Other points of contention are wages and the Minnesota hospitals' proposal to reduce the nurses' pension benefits.

The Minnesota nurses set their strike date of June 10 about 2 weeks beforehand, giving hospitals time to prepare. Maureen Schriner, a spokesperson for a trade group called Twin Cities Hospitals, said the 14 affected hospitals brought in 2800 replacement nurses on Thursday. Some hospitals rescheduled surgeries.
