You may be blocked if more than one person attempts to complete the survey from the same computer.  To avoid this, do one of the following:

1. Launch the email in your internet browser.  Look to the top of the email for "Having trouble viewing this email? Click here"  Simply click the link as indicated.  This will open the email in your internet browser and should allow you to complete the survey multiple times.
2. Go to www.gnba1941.com and in "Special Notes" you will see the surveys listed and simply click on that link.
3. Go to www.gnba1941.com and look for the red & white box below the picture "View Our Email Archives."  Click on that box and you will see the "2011 Information Request" email listed. 

Also, to facilitate this process, the intent is for you to complete the survey on-line -- not to print it down.
