Tri-Way and DSL Sports Team Physicals

Sunrise Medical Group will be in the campus PE room on Tuesday, September 13 from 3:00pm-6:00pm, offering sports physicals for any students that will be playing sports this school year.  The cost is $25.00, please make checks payable to Sunrise Medical Group.

Presentation times are:

5:45pm-6:15pm     Blackboard Presentation in the MPR      

6:15pm-6:45pm     Pre-School in classrooms   

6:30pm-7:00pm     Grades JK-2 in classrooms

6:45pm-7:15pm     Grades 3-5 in the classrooms

6:30pm-7:15pm     Grades 6-8 in the MPR

7:15pm-7:45pm     Blackboard Presentation in the MPR

All elementary and junior high students must wear WHITE shirts.  JK -3rd grade girls need to wear white shirts with their jumpers. Please make sure your child's hair is cut according to the school dress code.

Please bring a favorite lunch for you and your grandchild to share!

 

 *If grandparents are not able to attend, you may invite another special guest.

 

Please RSVP to your child's teacher by Friday, September 9th.

If no one is able to attend, please send a lunch with your child and we will make special arrangements so your child will not feel left out.
