Care4Life Now Open to All Patients in IC3 Beacon Clinics 

 

Many IC3 Beacon clinics are participating in an exciting research study to determine if a two-way text messaging program for diabetes coaching, education, and support improves HbA1c control. This new program, Care4Life, shows great promise in helping some patients improve management of their blood sugar over time. Participation in Care4Life was open only to a subset of patients who met certain criteria and who consented to participate in the research study. We are very pleased to announce that Care4Life is now open to all patients with type 2 diabetes in IC3 Beacon clinics, and is one more strategy for addressing the challenging task of moving HbA1c control measures. 

 

Riverton & Zenith Family Health Centers is a superstar recruiter for this innovative program. The clinic has integrated Care4Life enrollment into the clinic workflow and has been steadily enrolling patients over the past several weeks. <<<FULL ARTICLE>>>

Better Choices, Better Health - Diabetes

   

Better Choices, Better Health - Diabetes is a six-week online workshop. You can participate from your home computer at any time that's convenient for you.

 

Trained individuals who also have diabetes lead the workshop for about 25 participants.

When you sign up, you'll login to a special, secure website at least three times a week for a total of about two hours. While online, you'll be able to:

  • Find tips and tools to help you monitor your blood sugar, food, medication, and exercise - and even plan your weekly menu.
  • Create and track a weekly action plan to improve your health.
  • Discuss problems you're facing with other participants - and find solutions together.
  • Celebrate your successes!
