Getting Patients' Perspective is an Eye-Opener
The Institute of Medicine's special Committee on Identifying Priority Areas for Quality Improvement (2003) found that "...improvements in care coordination could result in significant benefits across the continuum of care across the life span," and subsequently recommended national focus on this "cross-cutting" topic. Staff from the Utah Healthcare Institute-St. Mark's Family Medicine, under Dr. John Berneike's leadership, are piloting a newly developed Beacon Diabetes Self-Management (DSM) Care Coordination Program.

A New Tool to Move Your A1c Control Measure: DiaLog, A Blood Sugar Tracking Game for the SmartPhone
In partnership with the IC3 Beacon, the developers at 4th Main Health have created a new diabetes tracking tool called DiaLog Unlimited. This smartphone application helps the user to track blood glucose, medication adherence, and diet and exercise goals in an engaging environment. Data entered by the user is summarized in a histogram, enabling the patient to assess trends and make associations with behavior. The game rewards timely testing, and is a great way to entice newly diagnosed diabetics to maintain regular blood glucose testing. The recordings can be exported to a PDF file that a user can keep as a record, and bring to office visits.
The application is free for both the Android and iOS (iPhone) market. Simply search "dialog unlimited" on your iPhone, or "dialog diabetes" in the Android Market.
