Community-Based Health Information Exchange in Nevada Gains Momentum 

 

Following the announcement and launch of the community-based health information exchange (HIE) in July 2011, HealthInsight, in partnership with the Nevada healthcare community and its stakeholders, has made great progress in moving forward. Most notable is the progress toward Proof of Concept by the collaborative spearheaded by Renown Health of Reno. The teams at OptumInsight, HealthInsight, and Renown did a fantastic job bringing up the feeds and monitoring and resolving issues. As of January 5, 2012, the following feeds are live:

  • ADT
  • Transcription
  • Radiology
  • Laboratory

The next step in the Proof of Concept is to begin the exchange of live data. . .

 

Beacon Clinics Have Major Impact on Diabetes Care 

 

HealthInsight has been collecting de-identified data from clinic electronic health records (EHRs) as it has become available since Beacon launched in 2010. Beacon analysts consider the 4th quarter of 2010 as the first data point for baseline measures. Specific clinic data has not been shared with anyone outside each clinic, although each clinic provides data to receive feedback on their own performance compared to a "community average." Many of you have wondered and asked about Beacon's impact in the community. 

 

Beacon's goal is overall community improvement rates of 10% to 20% in eight focus categories for patients with Diabetes Mellitus Type 2. We are pleased to report the following collective measure improvements . . .

HealthInsight Launches Learning and Action Networks in Utah

 

HealthInsight is pleased to announce the launch of our

Patient Centered Care in Action Learning and Action Networks in Utah, which will focus on improving cardiac health, promoting the use of electronic health records, and improving transitions of care between facilities and agencies or patients' homes. Learning and Action Networks (LANs) bring together communities to learn and explore ways to solve complex problems. This event will be held regionally throughout the state. 

 

We invite you to join your professional and regionally based colleagues as we jump-start our collaboration to provide the best in patient-centered care. As a Learning and Action Network participant, you will be making a difference in your community!

Meaningful Use Vanguards - Health IT Clinical Champions in Action

   

The national MUVers program was created to honor and bring together clinician champions who have made the transition to certified electronic health records (EHRs) and are successfully using their system as a clinical management tool. MUVers are 'Meaningful Use Vanguards,' meaning they have successfully implemented and are using an EHR system and are providing patient-centered and technologically advanced healthcare in their practice, their community and beyond.

 

HealthInsight identified the first Meaningful Use attesters in twelve regions throughout Nevada and Utah . . .

 

Partnering for Patient Safety 

 

The Nevada Hospital Association has contracted with HealthInsight to create the Nevada Hospital Engagement Network (NV HEN). This collaborative of Nevada hospitals is one of only 26 groups nationwide selected by the Centers for Medicare & Medicaid Services Innovation Center to work on the Partnership for Patients safety initiative.

 

Currently, several Nevada Hospital Association (NHA) members have committed to participate in the Hospital Engagement Network. Since membership in the HEN is not exclusive to NHA members, the HEN is also actively recruiting other short-term acute care, long-term acute care, psychiatric and rehabilitation hospitals within the state.

Utah Clinical Health Information Exchange- It's Your cHIE, Help it Grow 

 

The cHIE (chee)-Utah's Clinical Health Information Exchange-is growing. To date, about 41,000 consent forms have been collected from community members who want to take advantage of this new, powerful way to improve their healthcare quality and reduce costs.

 

The cHIE vision is to provide a secure, electronic, centralized way for healthcare professionals to view and share patient information with patient permission. The more healthcare professionals know about a shared patient, the better they can coordinate care and treatment. For example . . .

Juliana Preston to Serve as Executive Director for HealthInsight Utah

 

HealthInsight has announced that Juliana Preston, MPA, has been named as the Executive Director in Utah. Juliana has served as HealthInsight's vice president of Utah operations since 2009. Juliana joined HealthInsight in 1999 after several years in the long term care industry. She will be responsible for providing leadership and management to ensure that the mission, vision, values and goals of HealthInsight Utah and the HealthInsight Management Corporation are put into practice: effectively developing solutions to business challenges in the facilitation of sound organization and operational structures; supervising management staff; community relations; and supporting excellence in health care quality improvement to assure HealthInsight's programs and activities promote transformational improvements in healthcare.

 

 

Sharon Donnelly to Serve as Senior Vice President of Corporate Strategy

HealthInsight is pleased to announce that Sharon Donnelly has been promoted to Senior Vice President of Corporate Strategy. She will support Marc Bennett, President and CEO, in advancing strategic initiatives across the corporation and preparing for further growth. In this role she will ensure execution of strategic planning and alignment of the overall HealthInsight vision and strategy. Sharon has been with HealthInsight for fifteen years in roles of increasing responsibility.

Improving Transitions of Care to Reduce Hospital Readmissions: Effective Cross Setting Best Practices
