Choices Open Enrollment FINAL Update
Deadline to complete open enrollment is Tuesday, May 12. Even if you want to keep your choices the same, you must either submit a Choices enrollment form OR go online to re-enroll. If you do not complete online enrollment or submit a Choices form, your coverage will remain the same except you will not have a flexible spending account or the dependent fee waiver (if you qualify). Remember, if you are covering your spouse and/or dependent children for medical, dental, or vision you must submit the Employee Declaration or Tax Status Dependents form that was included in your Choices Open Enrollment packet. Essentially, if you do not claim your spouse and/or dependent children on your taxes, select "no" on the form; their portion of monthly premiums are taxable. Online enrollment is open for MSU - Great Falls employees through May 12. Instructions are posted under the Announcements at http://www.msugf.edu/facstaff/HR/index.php As always, if you have any questions regarding your benefit choices, please contact either Lisa Albert or Mary Kay Bonilla. |
