Join Us for a Free Conference on Health Care Transition for Youth with Special Health Care Needs!
 Are you the parent of a teen with special health care needs or disabilities? Do you or your teen have concerns or questions about the transition from child to adult health care? If, so this conference is for you!
Join us for a full day, FREE conference for parents and youth! Teens with special needs ages 12-17 are invited to attend with a parent. Special sessions, activities, and prizes for teens!
These day-long parent and youth conferences will offer information, ready-to-use planning tools, and resources for youth, families, and providers to make the health care transition process easier and less overwhelming.
