PATIENT STORY OF THE MONTH
 When I was diagnosed with a lethal virus twenty two years ago, the odds for survival that they gave me were one to three years. I was thirty four years old, an accomplished distance runner (3 NYC Marathons in the top 10%) smart, confident, and more than a little cocky, so I told them that in my case they had it wrong, and I have prevailed. It has not all been a bed of roses though, and in those twenty two years; I have been treated for many issues, by a wide array of doctors, and at countless institutions. Over the course of the past two months; I have had the extraordinary experience of being treated for two different issues at The Farber Center for Radiation Oncology. I went through the centers web site prior to my first visit, and as I read the patient stories; I though this is all well and good, but I am not really interested in an ethereal experience; I'm interested in getting efficient, and long lasting treatment for my cancer. I got that, and much, much more. During my initial exam, Dr Leonard Farber, the centers founder, noticed a small spot on the bottom of my left foot that two other doctors had missed in a full body exam. He didn't think it was an issue, but something to be watched. During that exam he was bending over looking at something, and looked up at me, and said "We're going to help you." There was something about his level of confidence that immediately led me to believe that he was correct. I knew when I walked out of the center that day-that this was where I wanted to be treated. Since I am in a position where I am responsible for a large portion of my medical co-pays; I called Vivian, the centers office manager, and told her that while I admired Dr. Farber's optimistic attitude that anything could be handled; I knew that she was the one that I would have to deal with on the financial issues. She told me that she had to get in touch with her billing people, and determine what type of plan could be negotiated, but closed that conversation by saying "If you want to come here, then you should come here, we will make everything work." I had no reason to doubt her. Treatment at the center was an unbelievable experience. I always arrived for my treatments a little early, and most of the time I was whisked away to one of the private dressing rooms to change into a terrycloth robe (no paper gowns here) before I could even get half way through my cappuccino latte espresso, one of the abundant choices of gourmet coffee's available for fresh brewing. The treatment team including Amy, Kim & Andrew was consistently extraordinary in their level of professionalism-with unerring attention to detail, and always quite sense of empathy.

I came to appreciate while in treatment here that there's absolutely nothing wrong with an ethereal experience when one is faced with daily treatment for issues that are so serious. In addition to the gourmet coffee & tea selections, the quiet music, the beautiful aquariums with exotic fish, the always calm atmosphere, the lovely gift bag after a few weeks of treatment-really does make a difference. The most important thing for me during those two months was that each and every time that I was at The Farber Center; I was always made to feel like 'THE" patient of the moment instead of the "NEXT" patient.
During treatment for my 2nd issue I developed an out of the ordinary side effect which led me to consider abandoning those treatments early since Dr. Farber's philosophy for that issue is to do a few treatments more once the problem is resolved so as to prevent recurrence. Dr, Farber made an alternate suggestion- that we stagger my last 4 treatments, 5 days off, and then 2 treatments, 2 days off, and then the last 2, which worked perfectly. During this experience, on a Sunday; I sent Dr. Farber an e-mail expressing my discomfort, and inadvertently listed the last two digits of my home phone number incorrectly. Unable to reach me by phone he responded to my e-mail four times in an effort to see if he could phone in a prescription that would make me feel better. E-mails and calls to his cell phone were always promptly responded to.
The overall attitude at the center is that the patients comfort is paramount, and anything else can be dealt with. I would suspect that if I were being treated at one of the major centers, the attitude would have been -this is your schedule, either finish it or don't- because we have our "NEXT" patient to get to.
Dr Leonard Farber is clearly a good business man, you don't run the type of world class treatment center that he does without being so, but more importantly he is one of those increasingly rare breed of doctors-who chooses to be a doctor first and foremost!

FROM THE EXPERT: Focus on
parotid enlargement

According to the Center for Disease Control, there are 1.2 million people in the US living with HIV and approximately 50,000 new cases are documented each year. Many patients will develop enlargement of their parotid glands, usually secondary to the development of benign lymphoepithelial cysts within the parotid, which can cause physical disfigurement (often described by patients as "chipmunk cheeks") and cosmetic distress. The classic clinical presentation is bilateral painless parotid swellings that are soft and slowly progressive in size.
Treatment options include close observation, repeat aspiration, HAART (highly active antiretroviral therapy), sclerosing therapy, radiation and surgery. Radiation has shown significant promise in reducing parotid gland size in those patients resistant to HAART, with some investigators advocating Radiation Therapy as the standard of care for this condition. Results with Radiation Therapy can be seen as early as after 4 treatments. Significant to complete improvement is typical by the completion of treatment, which is 3 weeks in duration. Side effects with Radiation Therapy are usually minimal and temporary.
