Therapy in the Pool? |

What is Aquatic Therapy?
By Barbara Birnesser, MPT
Even if your doctor did not specify aquatic therapy on your prescription, you surely have noticed it in our company's name and may have wondered what it is all about. It might be easier to begin with what aquatic therapy is not: it is not aqua aerobics or aqua arthritis classes, whirlpool therapy, or exercise dictated from the side of the pool. Essentially, it is physical therapy services provided in a warm water (88-92 degrees F) swimming pool, including impaired mobility access and hands on treatment. The definition established by the Aquatic Section of the American Physical Therapy Association is: the evidence-based and skilled practice of physical therapy in an aquatic environment by a physical therapist or a physical therapist assistant. Aquatic Physical Therapy includes but is not limited to treatment, rehabilitation, prevention, health, wellness and fitness of patient/client populations in an aquatic environment with or without the use of assistive or supportive devices and equipment.
Source: Aquatic Physical Therapy Section of the APTA http://www.aquaticpt.org/aboutaquaticpt/FAQ.htm What makes Aquatic Physical Therapy different than traditional (land based) PT are the following four principles: warmth, buoyancy, viscosity (water resistance), and hydrostatic pressure (force the water exerts on the submerged body). How we use these principles in our treatment plan depends upon the needs and goals of the individual patient. Warmth increases the elasticity of muscles and other soft tissues, allowing the patient to stretch more easily with less discomfort. It also promotes muscle relaxation by increasing blood flow which helps to reduce muscle spasms and "guarding" which can interfere with correct exercise performance. Additionally, the comfortable water temperature creates an environment of comfort and relaxation so you look forward to your treatment. Buoyancy reduces the weight bearing on the body (muscles, joints, and spine) up to 90% depending on the depth. This reduces pain and allows for increased movement with less effort. In addition, the constantly moving nature of water challenges patients' balance and postural strength.
Viscosity (or water resistance) provides the means for strengthening. When you push against the water, it provides 12 times more resistance than air. A ¼ mile walk in the water equals 1 mile on land. So while you feel lighter from the buoyancy, the exercises performed in the water are actually very challenging. You just don't think so until your session is finished!
Finally, hydrostatic pressure is the force of the water against the immersed body. It increases circulation, helps to decrease swelling, and stimulates joint receptors which help us balance. Now that you know a little more about what aquatic therapy is, here is a brief list of some of the conditions we treat in the pool:
Ankle, Foot, Knee, Hip Surgery Arthritis Back Pain/Surgery Cerebral Palsy Deconditioning Degenerated or Herniated Disc (Spine) Difficulty Walking Impaired Balance Multiple Sclerosis Muscular Dystrophy Obesity Parkinson's Disease Sciatica Stroke
